Page 2 of the Invitation to Bid lists the "First-Tier Subcontractor Disclosure (WH-179)" form along with the other forms that are included in the document. Was this form supposed to be included? Also, Figure No. 4 in the Geotechnical Data Report appears to be duplicated, and the "Exploration Location Plan" (Figure 3) appears to be missing. Thanks!

The 1st tier form has been added and the geotechnical investigation report has been updated to include the missing Figure 3.
